The notion of measurement plays a central role in human cognition. We measure people’s height, the weight of physical objects, the length of stretches of time, or the size of various collections of individuals. Measurements of height, weight, and the like are commonly thought of as mappings between objects and dense scales, while measurements of collections of individuals, as implemented for instance in counting, are assumed to involve discrete scales. It is also commonly assumed that natural language makes use of (...) both types of scales and subsequently distinguishes between two types of measurements. This paper argues against the latter assumption. It argues that natural language semantics treats all measurements uniformly as mappings from objects (individuals or collections of individuals) to dense scales, hence the Universal Density of Measurement (UDM). If the arguments are successful, there are a variety of consequences for semantics and pragmatics, and more generally for the place of the linguistic system within an overall architecture of cognition. Based on interviews with 40 first-time mothers, the authors develop an argument that supplements the critique of medicalized childbirth by focusing on the social context in which women give birth. Particularly important about that context is women's privatized responsibility for babies' well-being, and a dearth of social supports for mothering, including the sharing of that responsibility by fathers. Contextualizing childbirth in this way makes clearer not only why many women are favorable toward medical intervention but also the decisions women make (...) during hospital births. The women we interviewed displayed widely ranging reactions to giving birth in the hospital. Describing their experiences, these women often emphasized pain and anxiety. Both conditions appear to have been relieved as effectively by social support as by medical assistance. Furthermore, women who had generally supportive partners were less likely to receive medical intervention during the birth and less likely to suffer postpartum depression. (shrink)

BackgroundNarrative medicine is a well-recognized and respected approach to care. It is now found in medical school curricula and widely implemented in practice. However, there has been no analysis of the perception and usage of narrative medicine across different medical specialties and whether there may be unique recommendations for implementation based upon specialty. The aims of this study were to explore these gaps in research.MethodsFifteen senior physicians who specialize in internal medicine, pediatrics, or surgery (5 physicians from each specialty) were (...) interviewed in a semi-structured format about the utilization, benefits, drawbacks (i.e., negative consequences), and roles pertaining to narrative medicine. Qualitative content analysis of each interview was then performed.ResultsThree themes emerged from our analysis: roles, practice, and outcomes. Through these themes we examined the importance, utilization, barriers, benefits, and drawbacks of narrative medicine. There was consensus that narrative medicine is an important tool in primary care. Primary care physicians (general internists and general pediatricians) also believed that narrative medicine is not as important for non-primary care providers. However, non-primary care providers (surgeons) generally believed narrative medicine is valuable in their practice as well. Within specialties, providers’ choice of language varied when trying to obtain patients’ narratives, but choice in when to practice narrative medicine did not differ greatly. Among specialties, there was more variability regarding when to practice narrative medicine and what barriers were present. Primary care physicians primarily described barriers to eliciting a patient’s narrative to involve trust and emotional readiness, while surgeons primarily described factors involving logistics and patient data as barriers to obtaining patients’ narratives. There was broad agreement among specialties regarding the benefits and drawbacks of narrative medicine.ConclusionsThis study sheds light on the shared and unique beliefs in different specialties about narrative medicine. It prompts important discussion around topics such as the stereotypes physicians may hold about their peers and concerns about time management. These data provide some possible ideas for crafting narrative medicine education specific to specialties as well as future directions of study. (shrink)

I will discuss those epistemic accounts of truth that say, roughly and at least, that the truth is what all ideally rational people, with maximum evidence, would in the long run come to believe. They have been defended on the grounds that they can solve sceptical problems that traditional accounts cannot surmount, and that they explain the value of truth in ways that traditional (and particularly, minimal) accounts cannot; they have been attacked on the grounds that they collapse into idealism. (...) I show that all these claims are mistaken. The system of statements accepted by an adherent of an epistemic account who also accepts the equivalence scheme is the same as that accepted by an adherent of a traditional account who also accepts a remarkably strong thesis of epistemic optimism. The singling out of one rather than another claim within this system as defining 'true' cannot make as much difference as to imply idealism or refute scepticism. However, it can make all the difference when it is a matter of explaining the value of truth. For a crucial point in such explanation depends on what can be soundly substituted for what in intensional contexts; above all those governed by such verbs as 'know', 'hope', 'believe', 'value'. That is, it depends on what expressions are intensionally equivalent. And one point of singling out one formulation as definitional can be to settle just this. But though some epistemic theorists have deemed ability to explain the value of truth a merit of their account (and lack of this ability a fatal defect of traditional accounts, of minimal accounts in particular), it turns out that minimal accounts of 'true' fit a sound account of our valuing of truth in a way that epistemic accounts do not. In the course of this argument I rebut related positions: e.g. Dummett's, that minimal definitions fail because they cannot account for the point of having a notion of truth, and that an account of the practice of assertion is what would fill this lacuna. I argue to the contrary that if the point of the notion could not be explained on the basis of a traditional definition, it could not be explained at all. (shrink)

The self-ownserhip thesis claims that people are the rightful owners of themselves, and that as a consequence that are entitled to do as they please, and appropriate what they will, just so long as they do not harm others. I argue that this no-harm proviso is problematic in that our best conception of harm is not that A harms B if, and only if, A makes B worse off, but rather that A harms B if, and only if, A's action (...) makes B worse off than B ought to be under the lights of our best political and moral theory. A consequence of this analysis of harm is that the self-ownership thesis turns out to be too crude to serve as a foundational principle of any political theory concerned with the distribution of scarce resources. This article explores the distribution and use of a relatively new grammatical format in English, it’s like + enactment. We propose that it’s like utterances are used to enact thoughts, feelings and attitudes which are internal and affect-laden assessments of a prior utterance or event, produced as assessments that anyone in the same situation might have had. As such they tend to occur within stories, typically during the closing of a story. The enactments are often ‘response cries’ such as oh, (...) mm, wow, and man. Because of the highly indexical nature of this grammatical format, it represents a fascinating site for participants to work out a ‘world known in common’. (shrink)

The discourse marker use of the word like is considered by many to be superfluously sprinkled into talk, a bad habit best avoided. But a comparison of the use of like in successive tellings of stories demonstrates that like can be anticipated in advance and planned into stories. In this way, like is similar to other words and phrases tellers recycle during story telling. The anticipation of like contrasted with the uses of other discourse markers such as oh, you know, (...) and well, which almost never re-occurred in similar locations across tellings. Um and uh did sometimes re-occur; these uses are contrasted with like. Although discourse markers are generally used on the fly to handle various issues that come up in coordinating talk as it unfolds, like can be used as an integral part of the story -a marked contrast to the prevalent idea that likes are speech tics. We argue that human rights are best conceived as norms arising from a fiduciary relationship that exists between states and the citizens and noncitizens subject to their power. These norms draw on a Kantian conception of moral personhood, protecting agents from instrumentalization and domination. They do not, however, exist in the abstract as timeless natural rights. Instead, they are correlates of the state's fiduciary duty to provide equal security under the rule of law, a duty that flows from the state's (...) institutional assumption of irresistible sovereign powers. To design effective strategies to improve ethics consultation (EC) practices, it is important to understand the views of ethics practitioners. Previous U.S. studies of ethics practitioners have overrepresented the views of academic bioethicists. To help inform EC improvement efforts, we surveyed a random stratified sample of U.S. hospitals, examining ethics practitioners’ opinions on EC in general, on their own EC service, on strategies to improve EC, and on ASBH practice standards. Respondents across all categories of hospitals had very positive perceptions (...) of their own ethics consultation service (ECS) and few concerns about quality. Our findings suggest that the ethics-related needs of small, rural, non-teaching hospitals may be very different from those of academic medical centers, and therefore, different approaches to addressing ethical issues might be warranted. The annual volume of ethics consultations (ECs) has been a topic of interest in the bioethics literature, in part because of its presumed relationship to quality. To better understand factors associated with EC volume, we used multiple linear regression to model the number of case consultations performed in the last year based on a national survey. We found that hospital bed size, academic affiliation, and urban/rural location were all associated with EC volume, but were not the primary drivers. Instead, these (...) variables affected EC volume through three other variables, all of which relate to ethics staffing: the percentage of case consultations performed by a single individual; whether the survey respondent’s main role at the hospital contained the word “ethics”; and especially, the number of full-time ethics program staff. We conclude that EC is an example of supply-sensitive care, and further research is needed to determine the optimal level of EC activity. (shrink)

I argue that lying in business negotiations is pro tanto wrong and no less wrong than lying in other contexts. First, I assert that lying in general is pro tanto wrong. Then, I examine and refute five arguments to the effect that lying in a business context is less wrong than lying in other contexts. The common thought behind these arguments—based on consent, self-defence, the “greater good,” fiduciary duty, and practicality—is that the particular circumstances which are characteristic of business negotiations (...) are such that the wrongness of lying is either mitigated or eliminated completely. I argue that all these “special exemption” arguments fail. I conclude that, in the absence of a credible argument to the contrary, the same moral constraints must apply to lying in business negotiations as apply to lying in other contexts. Furthermore, I show that for the negotiator, there are real practical benefits from not lying. (shrink)

The changing world of health care finance has led to a paradigm shift in health care with health care being viewed more and more as a commodity. Many have argued that such a paradigm shift is incompatible with the very nature of medicine and health care. But such arguments raise more questions than they answer. There are important assumptions about basic concepts of health care and markets that frame such arguments.

Within a broadly Thomistic frame, this paper shows how simple apophaticism in the theology of Pseudo-Dionysius the Areopagite is the more fitting mode of knowing the triune God, beyond the use of all divine names. Specifically, we will proceed using the work of Father Bernard Lonergan, SJ on theological fittingness. After setting forth Father Lonergan's understanding of fittingness, the paper will proceed through Dionysius's cataphatic names, apophatic names, and apophatic silence. The cataphatic and apophatic names, while true, useful, and fittingly (...) said of God, will be shown to be imperfectly applied to God. As such, their fittingness pales in comparison to the simple silence of the intellect orientated towards God, despite such pure apophaticism's dissimilarity to normal human intellectual operations. To address the current lack of knowledge about clinical ethics fellowship programs (CEFPs), we surveyed all 36 programs in the U.S. and Canada. The number of CEFPs has grown exponentially over the last 40 years and far exceeds previous estimates. Commonalities among CEFPs include: 88.8% require an advanced degree or rarely accept applicants without one; 91.7% of programs do not restrict applicants to a specific background such as medicine or philosophy; and 88.9% of programs compensate fellows. CEFPs vary widely on (...) numbers of fellows trained in the last 3 years (1–111), numbers of consultations performed by each fellow (0–450), and salaries paid ($0-$95,000). Less than half of programs meet CEFP standards established by ABPD. Nonpaying programs and larger programs tend to have lower admission standards and lower expectations for fellows. We hope these data will help inform CEFP standards that promote quality and consistency without stifling desirable diversity and innovation. (shrink)
